Q:  How is my level of care determined?

An initial assessment is completed before moving in to determine what you need assistance with.  As your needs change, the director and nurse will work with you to determine what services you will need.

Q: Can I bring my own furniture?

Yes.  Each unfurnished apartment is equipped with a small kitchen table that has the capabilities of being removed for those tenants who wish to bring their own, no other furniture is provided.

Q:  What if I require more assistance than Lasata Crossings is capable of providing?

In the event that you begin to require more care than Lasata Crossings can provide, the Lasata Crossings Director and Registered Nurse will meet with you and your family to discuss a more appropriate care setting. 

Q:  Can I hire extra assistance from an outside provider?

Yes.  You may contract independently with outside providers as long as you 1) inform Lasata Crossings of such arrangements, 2) comply with the Lasata Crossings policies and procedures for Private Duty Caregivers, and 3) such arrangements are reflected in your Negotiated Risk Agreement

Q:  What is the availability of the nurse, and will I be able to keep my doctor?

The Lasata Crossing’s nurse will be available 40 hours per week.  All Personal Care Workers will be trained in how to handle medical concerns in the nurse’s absence.  The only requirement Lasata Crossings has regarding your doctor is that all tenants are required to have a designated primary care physician.
